We are want to be respectful of your time as well as make the most of your time spent with us. Prior to your visit, please print and complete the forms below and bring them to our front desk upon arrival along with your insurance information and photo ID.
DIRECT ACCESS TO THERAPY SERVICES
As of Sept 1, 2019, Texas now allows direct access to out-patient Therapy Services. You no longer need a referral to come see us, however we will remain in close contact to your primary care Physician so to allow you the best care possible using a team approach.
